Wallet-sized fold-up cards

The Diabetes Chart website has published wallet-sized cards featuring the Plasma Blood Glucose to HbA1c conversion table.  The cards, which also contain HbA1c facts and a lab results log, are available to interested support groups and health care providers at the prices listed below.

For those who feel it would be more cost effective to print their own cards, card templates are also available for purchase.
