This printable chart maker is a tool for
visually: tracking
glucose control, assessing current HbA1c
range, spotting problems, and setting goals. |

It opens in Microsoft Word (.doc
file). Users print a single monthly sheet and use a pen or pencil
to record daily readings as
dots within glucose ranges. The columns on the sheet are shaded to
form a histogram which estimates on-going HbA1c. The histogram can
also help users identify glucose control problems. |

The HbA1c Tracker works with both plasma
blood glucose readings and whole blood glucose readings. |
|
It can be set for three
levels of glucose control: "Tight", "Medium" and
"Loose". Diabetics set continuous improvement goals to
incrementally increase their control. | |
